A while back I used a roll of Fujichrome 64T type II film for a project in
my Photo 1 class. I found the box for the film and it says, ISO 64/19º.
Elsewhere on the box it says, ISO 50/18º. Does this have to do with the
temperature in which the film is used?

No. It's the logarithmic film speed (aka DIN rating)

Just one more question about this film regarding the color adjustments. The
entry for exposure and color balance correction for reciprocity failure says
that between 1/4000 and 1/30 of a second, "Not recommended." Does that mean
no compensation is necessary or that this film should not be used at all for
shutter speeds within this range?

They're saying that they expect the film to be used under tungsten
lighting, which will inevitably lead to longer exposure times so that's
what they've designed the film for.

There's nothing to say you can't use it faster than 1/15s but Fuji
isn't going to guarantee the results you get.
